Before your appointment, download and print the "Request for Background Check" form from the Sheriff's website. Fill it out completely. You must include your ORC code on the form. This is required by law. The WebCheck site will not pick a code for you. Bring it with you to the office at 200 West Crawford St, Findlay, OH 45840.
You need proof of identity. Acceptable forms include a state-issued photo ID, valid driver's license, military ID card, U.S. passport, or a Social Security card if the number is not on your other ID. Payment options are exact change in cash, a local Hancock County check, or credit card with a 3% fee (minimum $2 per transaction).

You also need to provide complete information about the agency that will receive your results. This includes the agency name, address with street number and city and state and zip code, fax number, and contact person. Under ORC 109.572, the explicit ORC number for your fingerprinting must be included on the form. This is required by law and the Sheriff's Office will not process your check without it.
